Sensory Over-Responsivity vs Under-Responsivity

Sensory over-responsivity means input registers as more intense than expected — sounds are louder, textures are harder to tolerate, light is brighter. Under-responsivity means input registers less, or later: a child may not notice being called, may seem unaware of temperature or minor injury, or may need more input before anything registers at all. They are opposite patterns, and the confusing part is that the same child frequently has both, in different senses. They can also look alike from outside — a child avoiding a noisy room and a child who did not notice you called both appear to be ignoring you.

The short answer

Over-responsivity is registering too much; under-responsivity is registering too little. They need opposite responses, which is why telling them apart matters.

What over-responsivity looks like

Input arrives more intensely than expected. Sounds are louder, tags and seams are intolerable, light is too bright, and busy places are exhausting rather than stimulating.

What under-responsivity looks like

Input registers less, or later. A child may not respond to their name, may not notice a scrape or that they are cold, and may need considerably more input before anything lands.

Can a child be both?

Yes, and it is common. Different sensory systems have different thresholds, so a child can be over-responsive to sound and under-responsive to proprioception at the same time.

Why they can look alike from outside

Because both can present as not responding.

A child who is over-responsive to noise may have withdrawn, gone quiet, or stopped processing language. A child who is under-responsive may simply not have registered that you spoke.

The distinguishing question: does the child seem distressed, or unaware? Over-responsivity usually comes with visible discomfort. Under-responsivity usually does not.

What about sensory seeking?

Seeking is frequently confused with under-responsivity, and it is not the same thing. Under-responsivity is not noticing. Seeking is actively going after input — crashing, spinning, chewing, squeezing.

They frequently occur together, because a child who registers less may pursue more to get the same signal. But a child can seek input without being under-responsive, and can be under-responsive without seeking. → Sensory seeking and craving

Why it changes from day to day

Because thresholds move. Tiredness, illness, stress, hunger, and how much the day has already demanded all change how much input registers and how much can be tolerated.

A child who managed the supermarket last week and cannot today is not being inconsistent. They are operating with less capacity.

Doing the wrong one makes things worse, which is why identifying the pattern matters.

Helping an over-responsive child

Reduce input at the source — quieter, dimmer, less crowded. Give control, including the ability to leave. Warn before predictable loud or bright events. Protect recovery time after demanding environments. Accept accommodations rather than pushing through them.

Helping an under-responsive child

Gain attention before speaking — name first, wait for them to look. Build movement and input into the day rather than adding it as a reward. Use more input to get the same signal: firmer touch, clearer contrast, more emphasis. Do not assume non-response means non-compliance.

What the evidence supports

Ayres Sensory Integration, delivered by an occupational therapist with specific training, in an appropriately equipped setting, with fidelity measures, has moderate-quality evidence for autistic children.

Environmental modification — changing the setting rather than the child — has performed more consistently than sensory input applied to a child, and it is the cheapest and most available option.

Weighted blankets and vests have repeatedly been found to have little or no effect on the outcomes usually claimed for them. Some people find them comfortable, which is a legitimate reason to use one, but comfort is different from a therapeutic effect.

Consider an occupational therapy assessment if sensory responses are affecting daily life, if your child is avoiding activities or environments most children manage, if mealtimes or dressing are consistently difficult, or if you are unsure which pattern you are seeing.
